Questions for My Doctor Journal

You sit down in the exam room, the doctor asks if you have any questions, and every single one you had all week is gone. Then you get to the car and remember all of them.

This is a small book for exactly that. One page per appointment, room to write the questions ahead of time, and room beside each one for what they said back. When somebody else drives you, they can write while you listen.

What you get

• A spiral bound journal, about 6 by 8 inches, that lies flat on a lap or an armrest
• Pages laid out for questions on the left and answers on the right
• A place at the front for the medication list, the diagnosis and the phone numbers you keep getting asked for
